Integrated Care Pathways (ICP) for MSK

What is ICP MSK?
ICP is a fully funded rehabilitation pathway designed for people with ACC injuries that are
likely to require coordinated rehabilitation and orthopaedic services. Vocational support
and other services to aid in recovery can be part of the ICP MSK pathway
Key features:
1. Fully funded pre and post operative physiotherapy management
2. Orthopaedic assessments pre and post operatively to guide the rehabilitation pathway.
3. Vocational Support for those who require support to return to work.
4. Other rehabilitation services: Psychology, Pain management assessment, Dietary support and Cultural supports
Testing
Physiotherapists record Outcome Questionnaires completed by the client and take Clinical Measurements to understand progress over time and how close the client is to achieving their goals. The ICP MSK goal is to achieve a safe and sustainable Return to Work or Return to Independence. Clients will also be supported to progress towards meaningful goals for them.
